Download the package above with all the following documents below and fill ALL of the forms for the purpose of submission:

1. MM2H Application form (Signature required)
2. Social Visit Pass Form (Signature required)
3. Personal Bond Form (Sponsor to sign and return back to us. Please leave it blank if you want Extreme Events MM2H to be the sponsor)
4. BK Form (Sponsor to sign and return it back to us. Please leave it blank if you want Extreme Events MM2H to be the sponsor)
5. Certified copy of passport
6. Five (5) Coloured passport sized photographs (3.5 x 5.0cm)
7. Certified copy of sponsor's identity card
8. Cover letter from sponsor (Please leave it blank if you want Extreme Events MM2H to be the sponsor)
9. Certified copy of medical report & RB II Form (To be done in Private Hospitals in Malaysia only)
10. Certified copy of Fixed Deposit (from any bank/financial institution in Malaysia)
    Individual: RM100,000.00, Couple: RM150,000.00
                                                         OR
    Certified copy of latest 3 months government approved pension funds
    (for applicants 50 years and above who apply using proof of monthly off-shore income)
    Individual: RM10,000.00, Couple: RM7,000.00
11. Certified copy of latest 3 months bank statement

Immigration to conduct an interview with Sponsor and Applicant

 

STEP 3:

Recommendation & Approval by Director of Immigration Department

 

STEP 4:

Pay MM2H Visa fees.
